How we verify fax numbers
Every number on this page was published by the agency itself. The two columns beside it say when that number was last confirmed, and what confirmed it.
The Verified by column says which of these it was:
Agency website the agency publishes this number. The date is the day we read it from their page or their published file.
ProntoFax user a fax sent through ProntoFax reached this number and the receiving machine confirmed it. The date is the most recent delivery.
ProntoFax automation we called the number and heard a fax tone answer. Nothing is sent and the call ends as soon as the line answers.
Needs verification recent attempts to this number did not go through. It stays listed because the agency still publishes it, but check it against your own letter before you send.

We never add a number because someone faxed it. A number appears here only if the agency published it. A delivered fax can confirm a number, but it can never create one.
What MassHealth accepts by fax
MassHealth takes applications and supporting documents by fax as well as by mail. These are the kinds of paperwork people commonly fax:
- A completed and signed application for health and dental coverage
- Annual renewal forms
- Proof of income, such as pay stubs or a benefit letter
- Proof of Massachusetts residency
- Documents a request-for-information notice asks you to return
Before you send
MassHealth publishes its own cover sheet, and these are the fields it asks for:
- The last four digits of the head of household Social Security number, or their initials and date of birth
- The name of the head of household
- Date of birth
- The MassHealth ID, if you have one
- The Reference ID, if you have one
- Your own name and phone number
- The total page count, including cover sheets
If your notice came with a barcoded cover sheet, MassHealth asks for the original rather than a photocopy. Photographing that original page and sending it as the first page of your fax is fine, because the original is what goes through the camera.
Use one cover sheet per household, and do not reuse a sheet issued for one household on another household paperwork.
Send the whole document in one fax. A signed application split across two transmissions can arrive as two incomplete ones.
Keep your delivery confirmation. If MassHealth has no record of your paperwork, that confirmation is your proof of what you sent and when.
