How we verify fax numbers
Every number on this page was published by the agency itself. The two columns beside it say when that number was last confirmed, and what confirmed it.

What IDES accepts by fax
IDES takes different paperwork at different numbers, and the ones above are what it publishes. These are the kinds of documents people commonly fax:
- An appeal to the Board of Review, to the number above
- A first appeal to a Referee, to the local office number printed on your Finding or Determination
- Documents for an appeal hearing, to the number on your Notice of Hearing
- A signed TAX-2 form to change withholding on a current claim
- A fraud or identity theft report
Before you send
Put a cover sheet on the front so IDES can match your fax to your claim. Include:
- Your full name as it appears on your IDES paperwork
- The last four digits of your Social Security number
- Your docket number, or the claim number printed on your notice
- A daytime phone number
- The number of pages, including the cover sheet
A first appeal does not go to the numbers above. IDES routes it to your local office, and that number is printed on the Finding or Determination you are appealing.
An appeal runs on a deadline counted from the mailing date on the decision, so send with time to spare rather than on the last day.
Keep your delivery confirmation. On an appeal it is your record of the date you filed, which is the date the deadline turns on.

Questions about faxing IDES
What is the IDES fax number?
It depends on what you are sending. IDES publishes separate numbers for an appeal to the Board of Review, for fraud and identity theft reports, and for a TAX-2 withholding change. All three are listed at the top of this page with the official source for each.
How do I fax an appeal to IDES?
A first appeal goes to the local office named on your Finding or Determination, at the number printed there. An appeal of a Referee decision goes to the Board of Review, at the number listed at the top of this page. Put your docket number on the cover sheet either way.
Can I fax my certification for benefits?
No. The IDES certification page lists online and phone only, and publishes no fax number for certifying.
How do I prove I filed my appeal on time?
