How we verify fax numbers

Every number on this page was published by the agency itself. The two columns beside it say when that number was last confirmed, and what confirmed it.

What NJ FamilyCare accepts by fax

NJ FamilyCare asks members to return a renewal packet and, in some cases, documents proving eligibility. These are the kinds of paperwork people commonly fax:

  • A completed and signed renewal packet
  • Proof of income, such as pay stubs or a benefit letter
  • Proof of New Jersey residency
  • Documents a notice asks you to return by a date
  • Changes to household size or income

Before you send

NJ FamilyCare does not publish a fax cover sheet of its own, so write your own first page and put these on it:

  • The name of the head of household, exactly as it appears on the notice
  • Date of birth
  • Your NJ FamilyCare or case ID, if you have one
  • The name of the form or packet you are returning
  • A phone number they can reach you on
  • The total page count, including the cover sheet

Renewals have a date on them. Send well before it rather than on the day, because a fax that fails at 4pm on a deadline leaves you no second try.

Send the whole packet in one fax. A signed renewal split across two transmissions can arrive as two incomplete ones.

Write your case ID on every page, not just the cover sheet, so loose pages can be matched back to your file.

Keep your delivery confirmation. If NJ FamilyCare has no record of your packet, that confirmation is your proof of what you sent and when.

Questions about faxing NJ FamilyCare

What is the NJ FamilyCare fax number?

The number published for renewal packets and verification documents is 609 631-6323, shown at the top of this page with its source and the date we checked it. If the notice NJ FamilyCare sent you prints a different number, use the one on your notice.

Why is this number not on the NJ FamilyCare website?

As far as we can find, New Jersey does not publish it there. We checked the NJ FamilyCare site, the application form, and the state renewal and contact pages, and none of them carries a fax number. The number comes from an Amerigroup provider bulletin quoting NJ FamilyCare’s own submission channels.

Can I fax my NJ FamilyCare renewal?

Yes. Fax is one of the three ways NJ FamilyCare names for returning a renewal, alongside mail and the online portal. Send every page in a single fax so it does not arrive split in two.

How do I prove NJ FamilyCare received it?
