SSA-44 fax number: send your IRMAA appeal to your Social Security office

There is no national fax number for the SSA-44. Social Security says to fax or mail the completed form and your evidence to a Social Security office, so the number you need is your local office’s. Find it with our Social Security fax numbers by state or with Social Security’s office locator, both linked below.

ProntoFax is an independent fax service. We are not affiliated with, endorsed by, or connected to the Social Security Administration or Medicare.

What IRMAA is

If your income is above a set level, you pay an extra amount on top of your Medicare Part B and Part D premiums. Social Security calls it the income-related monthly adjustment amount, or IRMAA.

If a life-changing event reduced your household income, you can ask Social Security to lower it. The form for that request is the SSA-44.

The seven life-changing events on the form

The SSA-44 (12-2025) lists seven events: marriage, divorce or annulment, death of your spouse, work stoppage or reduction, loss of income-producing property, loss of pension income, and an employer settlement payment.

Premium notices usually arrive in late November and December, which is when most people learn about a higher amount. That is general timing, not a date Social Security publishes, so go by your own notice.

How to send it

Social Security’s instruction is: “Fax or mail your completed form and evidence to a Social Security office.” You can also sign in to your Social Security account and upload it there.

The form itself prints no fax number and no mailing address. Send the form and your evidence together, in one fax, to your local office.

Find your office

Look up your office with Social Security’s office locator, then check its fax number on our Social Security fax numbers by state, linked below with the date we read each one. If a letter from Social Security gives you a fax number for your case, use that one.

What to attach for each event

Page 8 of the form lists the evidence Social Security wants for each event. For the most common ones:

  • Marriage: an original marriage certificate, or a certified copy of the public record of marriage
  • Divorce or annulment: a certified copy of the decree of divorce or annulment
  • Death of your spouse: a certified copy of the death certificate, or of the public record of death
  • Work stoppage or reduction: a signed statement from your employer, or copies of your pay stubs
  • Loss of income-producing property, loss of pension income, or an employer settlement payment: the evidence page 8 lists for that event

Before you send

Put a cover sheet on the front so the office can match your fax to your record. Include:

  • Your full name as it appears on your Medicare card
  • “SSA-44, request to lower IRMAA”
  • A daytime phone number
  • The number of pages, including the cover sheet

The form says: “We must see original documents or certified copies of evidence that the life-changing event occurred.” A fax delivers a copy, so for a marriage, divorce, or death, Social Security may still ask to see the original or certified copy after your fax arrives. Keep it ready.

Keep your delivery confirmation. If the office has no record of your request, it shows what you sent and when.

Questions about faxing the SSA-44

Is there an SSA-44 fax number?

Not a national one. Social Security says to fax or mail the form and evidence to a Social Security office, and the form prints no fax number of its own. Use your local office’s number.

Can I fax my evidence with the form?

Yes, and Social Security asks for the form and evidence together. For a marriage, divorce, or death, the form also asks to see original documents or certified copies, so the office may follow up to see them.

When do IRMAA notices arrive?

Premium notices usually arrive in late November and December. That is general timing rather than a date Social Security publishes, so go by the date on your own notice.

How do I prove I sent it?

ProntoFax emails you a receipt once the receiving machine confirms delivery, showing the date, the destination, and a link to view the document you sent.

How much does it cost?

$2.99 for one fax of up to 25 pages. No subscription and no account. Your card is held and only charged once delivery is confirmed.
